Your first appointment

Whether you have been referred to our office by your primary care physician, or are seeking a second opinion, we at Hematology & Oncology Consultants, P.C., want to make sure your appointment with our office goes as smoothly as possible.

What to Expect

Prior to your first visit, your referring physician will send us the necessary records and information in order for us to best serve you. You will most likely have blood drawn, and then a nurse will review your medication list and obtain your vital signs. One of our health care providers (Physician Assistant, Nurse Practitioner or Physician) will do an initial interview to discuss your specific situation and symptoms, and perform a physical exam. You will then spend time with the physician discussing your treatment options. Before you leave, you will have an initial plan with any additional testing scheduled, as well as a follow-up appointment. We recognize that your first visit can be very stressful, so we encourage you to bring someone along to assist you in listening to and understanding the information you will receive.

What to Bring

To help ensure that we have the most up-to-date and accurate information, please bring the following items to your appointment.

  • Your driver’s license or photo ID
  • Your current insurance card(s)
  • Your co-payment, if your insurance requires one
  • A list of the current medications you are taking
  • A list of your allergies
  • The names of the other physicians who care for you

You may print and complete the forms listed below and bring them to your appointment as well. Otherwise you will be asked to complete these when you arrive.

If you were not referred to us by your physician, please fill out our Patient Self-Referral Form, and be sure to bring your medical records to our office at least 24 hours in advance of your scheduled appointment time.
